Parent Notification
Anytime a child is tested on the EASI, the parent or guardians must be given the Parent Notification Form (available in the test sheet folders for each language as document 00-a Parent Notification). This form notifies parents and guardians that it is possible that a child’s deidentified scores could be used in future research projects and gives them the opportunity to opt-out.
It may be useful to parents and guardians to provide them with the document found in the ESP under About the EASI (currently available in the ESP) titled: Description of the Evaluation in Ayres Sensory Integration Tests (EASI)©For Parents and Caregivers.
